There are many factors to consider when choosing a breast augmentation. Many patients focus on the size, shape and type of implant (which are very important factors) - but often the patient does not consider the incision technique. There are 3 common incision sites; within the breast fold (inframammary), around the nipple (periareolar) or under the arm (transaxillary).
An experienced breast augmentation surgeon should be able to discuss in detail each procedure and recommend the best option for your particular situation. Here is a brief description of each:
- Inframammary - This is a very common incision that is made in the fold of the skin underneath the breast. Most surgeons would agree that this is the ideal method for providing symmetry. However the scar is more visible than periareolar or transaxillary.
- Periareolar - This incision is made around the nipple and is typically the most discreet incision. When performed by an experienced breast augmentation surgeon, the incision should be nearly undetectable after it has completely healed.
- Transaxillary - This incision is made under the arm. Many patients prefer this method because there is ZERO scarring on or around the breasts.

The demand for non-surgical aesthetic procedures has continued to grow at a rapid rate over the last decade. These high technology aesthetic procedures include ultrasound, radiofrequency, lasers, and various heating and cooling treatments. They all promise significant rejuvenation of skin, reduction of fat, and even the elimination of cellulite with no downtime.
In most cases the before and after pictures are produced by the companies that have spent millions of dollars in developing these devices. Most of these minimally invasive no downtime procedures require multiple treatment sessions which cost thousands of dollars and the results are often mild at best or non-existent. One of my mentors Dr. Jeffery Klein MD, the inventor of tumescent liposuction (which revolutionized cosmetic rejuvenation worldwide), told me in 2007 that in his opinion ” half of what you see advertised in the arena of minimally invasive aesthetic/cosmetic procedures is nothing more than hype." I have always remembered that statement must say that after doing my own research over the years that I agree.
Our cosmetic center has invested heavily in many different first tier lasers, light, radiofrequency and ultrasonic systems. Several of these devices do have minimal downtime, some have significant downtime, but they all produce visible real results. We document these results in our website gallery with before and after pictures. We are adamant that we will not invest in equipment that does not produce real and definitive results.
My advice to patients seeking any aesthetic procedure is to ask your physician to see multiple before and after pictures of patients that he or she has actually performed the procedure in question on. The photographs should be standardized in terms of lighting, positioning, and camera angle with a consistent background. When evaluating neck treatments for example, make sure that the position/angle of the head and neck in the before and after picture is identical. A subtle way to significantly improve the appearance of neck rejuvenation is to increase the jaw/neck angle by tilting the head back slightly. If your physician does not have his or her own photos available or the photographic results do not live up to the sales pitch, beware.
Finally before plunking down your hard earned money it is also a good idea to talk to other patients that have had the same procedure performed to make sure that they are pleased with their results.
